Functional home assessments delivered by Legacy clinicians certified as Senior Home Safety Specialists® through Age Safe® America
RALEIGH, NC – November 3, 2026 – Legacy Healthcare Services today announced the launch of Safer at Home, a new functional home safety assessment program designed to help active adults maintain independence, reduce potential safety risks, and continue living confidently in the place they call home.
The program is the newest addition to Legacy HealthSpan, the organization’s comprehensive continuum of care focused on supporting the health, wellness, independence, and quality of life of active adults. Legacy HealthSpan brings together onsite rehabilitation and wellness programs in senior living communities, private-pay home care services, Better at Home mobile outpatient therapy, Medicare’s GUIDE program for dementia care support, and now Safer at Home functional home safety assessments.
As more active adults choose to remain in their homes, creating safe and accessible living environments has become an increasingly important part of healthy aging. According to AARP’s 2024 Home and Community Preferences Survey, 75 percent of adults age 50 and older want to remain in their current homes as they age. The Centers for Disease Control and Prevention identifies unintentional falls as the leading cause of injury and injury-related death among adults age 65 and older.
Delivered by clinicians certified through Age Safe® America, Legacy Healthcare Services’ Safer at Home program provides comprehensive home safety assessments conducted by professionals who have earned the Senior Home Safety Specialist® (SHSS) certification.
During each visit, certified clinicians conduct a structured, room-by-room assessment, evaluating potential safety concerns involving flooring, lighting, furniture placement, bathrooms, kitchens, stairways, walkways, porches, decks, and fire safety equipment.
Using the Age Safe® app, clinicians complete a 240-point assessment and provide an Age Safe® Home Score™ report identifying potential hazards and prioritizing practical recommendations. These may include improved lighting, grab bars, assistive equipment, and home modifications to help create a safer, more functional living environment.
What distinguishes Legacy Safer at Home is the clinical expertise behind each assessment. Legacy Healthcare Services professionals bring firsthand knowledge of mobility, balance, physical function, and everyday activities, allowing them to evaluate not only the physical environment but also how individuals interact with and navigate their homes.
“I started as a staff physical therapist in 1994, so I know what our clinicians see when they walk into a home. I went through the certification myself. Spotting the hazard was never the hard part. The hard part was that there was no consistent way to record it, rank it, and provide families with something they could act on.
Safer at Home gives them that, and it does not take another visit from another specialist,” said Carl Shrom, MS, PT, Chief Executive Officer of Legacy Healthcare Services.
The program also includes post-hospitalization safety checks and caregiver education, helping individuals and families address potential concerns during important transitions in care.
“Falls are the leading cause of injury for adults over 65, and aging in place only works if the home works. Legacy built a real service around that and certified their own workforce to deliver it. We are proud to partner with them, and congratulations to the whole team on the launch,” said Dan Davenport, Chief Executive Officer of Age Safe® America.
With the introduction of Safer at Home, Legacy continues to strengthen its integrated approach to supporting active adults wherever they call home. By bringing together rehabilitation, wellness, personalized home care, mobile outpatient therapy, and proactive home safety solutions, Legacy Healthcare Services helps individuals maintain physical function, preserve independence, and enjoy a higher quality of life.
Safer at Home is available now. Active adults, families, and referral partners can schedule an assessment by calling 888-654-1948 or emailing intake@legacyinc.com.
